When someone you love has an incurable illness and treatment can no longer control it, there is still a lot you can do. Even when life cannot be prolonged, comfort and symptom relief are almost always possible—and that is the heart of hospice care.
Many people prefer to spend their final months at home. Hospice comes to you. It can also be provided in nursing homes, personal care homes, or other facilities where your loved one lives.
Once your loved one begins hospice, you will receive regular visits from our team and have access to support 24 hours a day. If your loved one becomes too sick to stay at home, we have a beautiful facility that provides round-the-clock care.
A compassionate team will guide, support, and assist your family every step of the way. There is almost always something that can be done to improve the quality of life near the end of life. That is what hospice does: it provides comfort and dignity for your loved one, and peace of mind for you and your family.
An E-kit will be delivered to your residence (for home care patients) upon admission. It is a small kit containing medications for the management of symptoms that may not be currently present but could occur—like nausea, vomiting, pain, or shortness of breath.
Durable Medical Equipment (DME) is provided as outlined in your Plan of Care and is covered by Medicare and Medicaid. Equipment may include oxygen tanks, shower chair, hospital bed, etc.. Your nurse or physician will recommend what you need and arrange home delivery. If you need additional items, repairs, or replacements, contact your hospice nurse or our 24-hour line at (678) 962-5850.
Hospice also provides and delivers medication related to your hospice diagnosis seven days a week through our contracted pharmacy.
Physical, speech, and occupational therapy may be available as deemed appropriate
*If families request, our chaplains are happy to officiate or participate in memorial services, if available.
